This is called "speaking to your base". The message is that Obama is just another dissolute, immoral, tax & spend liberal out of touch with the Real America, and is heading like all overexposed celebrities, for disaster and disgrace.
Which is like calling McCain a war-mongering heartless old man out of touch with the 21st century, headed for world holocaust. It's tripe, but emotionally effective tripe.
The reactions to this ad of those who do not lean towards McCain already are irrelevant. It's not meant for you.
A satire that does not cut is not satire. I think the magazine is guilty of bad taste, but no more. They knew exactly what they were doing, stirring it up just enough to get mentioned in the press.
More worrying to me is the notion of "responsible" versus "irresponsible" journalism. That's a very dangerous idea because it is both emotionally charged and ill-defined, like 'politically correct'.
The punishment is brutal. One could argue that having to live in prison for 30+ years with everyone knowing what you had done would be a worse punishment. On the other hand, the death penalty is the cheaper option.
