Dorland Health, a division of Access Intelligence, announced the winners of the third annual Case In Point Platinum Awards, recognizing outstanding case management professionals and organizations committed to ensuring the system is patient- and family-centric, safe and effective.
The Awards Program highlighted the innovative and creative work across the healthcare system that is improving access, promoting wellness and ensuring safe transitions of care through effective care coordination across the care continuum.
A distinguished panel of judges reviewed entries across 23 categories that encompassed care coordination efforts in diverse settings, including acute care, managed care, disease management, behavioral health, social work, rehabilitation, workers’ compensation, military healthcare, wellness and prevention programs and others.
”All eyes are on the practice of case/care management as a way to improve efficiency and effectiveness in today's healthcare system,” said Anne Llewellyn, editor in chief of Dorland Health and chairwoman of the awards ceremony. “This Platinum Awards Program, like no other awards program in the industry, provides the opportunity for organizations and professionals to celebrate their successes in a way that elevates the practice.”
The ceremony bestowed formal recognition to winning organizations, as well as honorable mentions that are making a difference in the lives of all who utilize the healthcare system.
